Analysis of Gastroscopic Findings in Resected Stomach

Abstract
For evaluation of the status of resected stomach, gastroscopic examination was done in 36 patients who received total or subtotal gastrectomy, from March 1980 to September 1982 at Chonnam National University Hospital. The results were as follows
1) The patients were composed of 31 males and 5 females and the distribution of age was in the range from 21 to 71.
2) Operation diagnosis were stomach cancer (52.8%), duodenal ulcer (36. 1 % ) & gastric ulcer (11.1 %). Billroth I type operation was 11.1 % & Billroth I I type operation 88-990".
3) Endoscopic diagnosis of resected stomach were gastric cancer (84%), gastritis (5%) and normal (11%) in preoperative gastric cancer group, ulcer (38%), gastritis (54%) & normal (8%) in preoperative duodenal ulcer group and ulcer (25%), gastritis (50%) & normal (25%) in preoperative gastric ulcer group.
4) Most frequent clinical symptom was upper abdominal pain and other clinical symptom & sign were upper abdominal mass, indigestion, occult blood, weight loss, etc. in order of frequency.
5) The time interval from operation to gastroscopic examination was in the range from 2 month to 5 year, most frequency was under 1 year duration (69.4%).
6) Clinical stage at the time of operation of recurred cancer were in stage II (6.3%), stage III (87.5%) and stage IVa (6.3%). Operative method were total gastrectomy (38%) & subtotal gastrectomy (62%). The type of recurrent gastric cancer by endoscopic finding were polypoid (31.3,%), ulcerative (43.8%), infiltrating ulcerative (18¢¥,8%) and diffuse ulcerating (6.3%).
The site of recurred ulcer by endoscopic finding were marginal ulcer (50%), gastric ulcer (33.3%) and jejunal ulcer (16.7%).
7) The frequency of postoperative gastritis were 25% in Billroth I operation & 61.5% in Billroth II. Gastritis were compsoed of superficial gastritis (20%) & atrophic gastritis (80%), most of all cases showed over the degree of moderate erythema which were localized around the anastomosis.site (60%) and dispersed in whole resected stomach (40%).
8) Radiologic diagnosis was possible in 75% of recurrent gastric cancer, 16.7% of marginal ulcer and 40% of gastritis which was diagnosed by endoscopic finding.
